Does the occasional Rhino have a third horn?
Yes, they do. Its rare but not unknown.

There was a curious case reported in the International Zoo News at (I think it was) Lisbon Zoo some years ago where a Black rhino was photographed with a small 'third horn' that it was suggested had somehow been sewn onto the animal as a joke. However, it was in exactly the same place as the Paignton animal's is, so maybe it was natural and just looked like a hoax.
 
There was a black rhinoceros at Magdeburg Zoo in the 1980s that had a small third horn.
