It means any changes made require Listed Buildings Consent from the local Planning Authority (harder to get than normal planning permission). Whether changes are too severe is a judgment they make on a case by case basis, but really dramatic changes are unlikely to be approved without a very strong reason.
There are occasionally controversies with local authorities seemingly setting such stringent requirements on renovations that the building literally starts falling apart because no-one can afford to do what they want.
