I'm pretty sure there is another species of frog that did, but I can't recall which nor where

potentially on the block
Fernandina Island Galapagos tortoise -Darwin research station (only 1 known purebred)
Yangtze giant soft shell turtle -Suzhou Zoo (2-3 left in the wild)

its not a zoo but the torties are on display. the other two did die in captivity (and I assume the deer was on display ) so the judge will allow. :)

Rabb's treefrog -Atlanta zoo
The last Rabb's fringe-limbed tree frog died at the Atlanta Botanical Garden, not Zoo Atlanta, in 2016. Zoo Atlanta however, had the second-last Rabb's frog in the world though, which died in 2012.
Also, the last Pinta Island Tortoise died at Galapagos National Park (in 2012), and the Northern White Rhinoceros is on the chopping block with just two females left at Ol Pejeta Conservancy in Kenya.
