Perth once sent quokkas to Ueno Zoo

(now closed, since 2002) Bannamah Wildlife Park in Dunsborough 250km drive south of central Perth captured some mainland Quokkas for their park in the early 1970s (the owner Aubrey Chugg was made an honorary national parks and wildlife ranger and was obviously permitted to obtain them), not sure if they ever sent some of those mainland Quokkas to Perth Zoo perhaps (Bannamah in turn got Perth Zoo's Wedgetailed Eagles in the late 1980s so there was definitely some exchange documented between the two).

According to this news, Perth sent quokkas to Ueno Zoo in April 1961. Was it from Perth Zoo?

It could have been, or they could have been wild caught.
Considering the time and age, these individuals being wild caught can't necessarily be easily ruled out. Especially considering the quokka's very limited natural range hinging on Perth itself.
