I think the Dallas Zoo has had the most name changes. If I'm wrong, please let me know.
Here's the evidence as I know it :
The Dallas Zoo was first known as the City Park Zoo, starting in 1888, when it was located at City Park. Coincidentally, the park's name was changed to Eakin's Park, then Sullivan Park, then Old City Park, then Heritage Village. But now I'm getting off-topic. When the zoo was transfered from City Park to Fair Park in 1910, it was renamed as the Fair Park Zoo. In 1912, it was sent to Forest Park in Oak Cliff and renamed the Forest Park Zoo. This should not be confused with the Forest Park Zoo in Fort Worth, which was eventually renamed the Fort Worth Zoo. Forest Park was originally called Oak Cliff Park, then became Marsalis Park, then Old Marsalis Park. But after Oak Cliff was annexed to Dallas in 1905, the city eventually changed the name to Forest Park in 1909. The Forest Park Zoo remained that way until the area schools and the Oak Cliff Chamber of Commerce convinced the city council to change the park's name to Marsalis Park. And so, the Forest Park Zoo became the Marsalis Park Zoo. However, most people just called it the Marsalis Zoo. But then management chose to change the name again to the Dallas Zoo At Marsalis Park. This time, most people simply preferred to call it the Dallas Zoo. In 1983, management finally decided to get with the program and shortened the name to the Dallas Zoo as part of a new marketing campaign.
And there you go, folks. That's six name changes.
