I don't mean to hijack this thread, but I compiled all the comments with Great Argus' fantastic original list. It took me awhile so figured I would post here for everyone's benefit. Disclaimer: I didn't confirm the information in the posts myself.
@KatVet : where are you getting the information about Cleveland still having lorikeets? As far as I know, they only have budgerigars now. The lorikeet aviary at Cleveland became the budgerigar aviary. There might still be some lorikeets off exhibit, but I doubt it. Just budgerigars as far as I know...
Additionally having now done a full run through of the list you've posted I count offhand well over two dozen errors and additions/removals - more reason for asking instead of jumping in uninvited.

No problem!!! Last year (Australian Adventure- the part of the Cleveland Zoo where the lorikeets were- is open from April to October) they did have lorikeets, but, now, only budgerigars. The website lists that they still have the lorikeets, but I have been to the zoo 4 times this year and can attest they only have budgerigars now.
Theater of the Sea has blue-and-gold macaw, scarlet macaw, military macaw, hyacinth macaw, yellow-collared macaw, African grey parrot, salmon-crested cockatoo, and white cockatoo
Wild Adventures Theme Park in Georgia has African grey parrot, blue-and-gold macaw, green-winged macaw, millitary macaw, budgerigar, rosella (unsure of what species), Patagonian conure, and scarlet macaw (all listed from the website).
*These birds were originally from Woodland Park Zoo in Washington, which does not make them a holder of those species anymore.
