San Francisco Zoo:

First visitor killed by an escaped animal at an AZA Zoo 2007

Positive.

San Diego and San Francisco had permanent exhibits of koalas beginning in the 1960s, and San Diego began regularly breeding them in the 70s and has been ever since. It's possible Columbus was the first inland US zoo (i.e. located nowhere near any eucalyptus trees) to permanently exhibit koalas.
 
The Toronto Zoo was also the first zoo to produce young in 1994. That might be in Canada though? First birth of Tasmanian devils in North America in 1995.
 
Jacksonville Zoo

First zoo to successfully breed Leadbeater's Ground Hornbills and Toucans in North America.

Not a "First" but every black Jaguar in North America until as recently as the mid 2000's could be traced back to Zorro, a jaguar kept here.
 
Reid Park Zoo (Tucson, Arizona): First zoo-based construction project to receive a Platinum level (the highest) LEED certification from the US Green Building Council. Awarded early in 2009 (building opened in 2008) for our Conservation Learning Center.

The Toronto Zoo is the first zoo to have a completly geothermal run, outdoor exhibit.
 
They also developed scientific animal diets now used throughout the world and pioneered conservation programs for Massasauga rattlesnake, karner blue butterfly and redside dance.
 
Hasn't happened yet, but Taronga zoo here in Sydney is well on the way to becoming the first to breed sea leopards. (Keep your fingers crossed!)

Bioparc in Valencia the first in Spain to display owl-faced monkeys, fossas, aardvarks and 7 lemur species in a walk thought exhibit. (it´s a poor balance I known :D)
