Squrill monkey, not sure I'd class as attack but jumping on me and biting while cleaning out.
I was bitten by a Common Marmoset on my little finger at Twycross Zoo and boy did it hurt.
Yesterday! Got too close to a nest of paper wasps and received 5 painful bites! Over the course of 30+ years of working with animals received many scratches and bites, whilst managing to avoid serious injuries from several species including elephants, hippos, giraffes, deer, red kangaroos, ostriches, cassowaries and several monkey species etc., etc., although usually by pure luck!
I've been stung by a wasp a couple times.
Literally nothing else, I guess wild animals love me or something!

It pecked me while seeking for food in hand)
Many years ago at a church barbecue, I was being a dumb child and started messing with a box turtle. These turtles would normally hide in their sealable shell but I managed to make this one mad enough to open its mouth. I stupidly placed my finger in it's mouth and it clamped down. Someone eventually pried the turtle's mouth open and let the turtle go. It luckily didn't break my skin, but my finger hurt and the ordeal embarrassed me enough that I never did anything like that again.
Not entirely related to Breckenridges post, but in PN at family gatherings, we used to have a full-on swing set and slide in our backyard, and American toads would constantly be found underneath the slide. Anyway, one day I had kids over, and when showed everyone the toad they all ran and kept thinking They would get warts 
So I picked up the toad and some stupid kid (will not state names) ran towards me while I was carrying the toad and full almost wacked the toad out of my hand. That poor toad, he was okay though, I think he jumped just before his hand collided with the toad. I got a blow to the face though because the little kid missed and smacked me in the face instead.

I have been chased off by nesting Southern Lapwings. Those things are nasty...
Blue Tits are known in some places as ‘Billy Biters’. I realised how appropriate this was, when ringing some. They get hold of the skin next to your finger nails, and really chew hard. They don’t actually come back at you when released, but I wouldn’t be surprised if they did

I was hiking in a state park with my family and saw something I thought was a toad. So I reached to pick it up to show it to them but it was a copperhead and it jumped and bit my sleeve, then slithered away. I was startled and jumped too.
Probably best not to pick any animal up, for both your and the animal’s safety, let alone a copperhead...
In all seriousness though, it was likely a dry or warning bite but nevertheless a very dangerous situation.
I pick up herps and insects occasionally - I'm always extra careful to make sure not to harm the animal - and that I know the species is not venomous.
I never handle these animals for more than a minute or so and release them afterwards.
After several days of petsitting at a farm, which wasn't even my first time around their current birds, this Polish rooster decided he hated me and kept charging me. He got a few good gouges into my right leg before I was able to start blocking him (first time I thought it was an accident, and I was also carrying several things of food). Still ate the food I put down Had to spend the rest of the week taking the big metal pole in with me or quickly shutting the coop doors if he was inside (there's a small run and a large run, both connected to the same coop, and he'd try to run through the coop to the other yard if I was switching).
