Are These in Captivity?

I have seen 2 deep water lizardfish species in Japanese aquariums, but I’m wondering why lizardfish as a whole aren’t more common in aquariums both public and in the home?

They're ambush predators that rarely take anything but livefoods, making them more difficult for home aquarists at least. They're also relatively sluggish and frequently not particularly colorful.

Are these in captivity?

Galapagos shark
Pygmy seahorse
Helmeted hornbill (I know there were some in Bali, but are they still there? Why do they do poorly in captivity?)
Tucuxi

Tucuxi

Tucuxi were kept at Munster until relatively recently. Helmeted hornbill has been held at Walsrode and London a long time ago. :)

I know Monterey used to have them, but now I can only think of Mandalay Bay Aquarium having them in Vegas.

There are Muricella paraplectana at Steinhart Aquarium, but I don't know if they're on exhibit, this is based off of this article.

Baby Pygmy Seahorses Are Even Cuter Than You Think

Last visit early 2020 and I didn't see them but I have heard of behind the scenes breeding success. I could have missed them easily since there are so many small aquariums in Steinhart.

How about these?

Hawaiian Crow (in a setting open to the public or by appointment)
most surviving Hawaiian honeycreepers
New Caledonian Crow (I know some labs study their tool use)
Megapodes other than Maleo, Brush Turkey, and Tongan.

As far as public collections go:
Hawaiian Crow is kept at Pana'ewa Rainforest Zoo.
No Hawaiian honeycreepers are kept.
New Caledonian Crow is kept by Nouméa Zoo.

Were any Hawaiian honeycreepers ever kept? As a kid I remember signs for them at the Smithsonian National Zoo, but didn’t see any.

Yes, SNZ used to keep Iiwi at least, possibly others. I know Philidelphia had one species recently too, I forget which one. They used to be in Hawaiian collections, too. There just aren't any captive Hawaiian honeycreepers on public display now.

Hawai'i Amakihi was kept by Philadelphia until August 2014 when they last female died. I managed to see her roughly a month beforehand.

Are there any Southern Bluefin Tuna at aquariums? They are aquacultured in Australia so I wouldn't be surprised if an Australian aquarium had some, but large open ocean tanks are not common in Australia, from what I have heard.

I don't know about Southern, but Bluefin Tuna of some type have been kept at Monterey Bay and some Japanese aquariums, so I don't think keeping Southern would be too difficult. Maybe slightly different water conditions or temperatures, but otherwise it should be possible.

Are the following amphibians in captivity?

Dicamptodon giant salamanders
Tailed frog
Darwin’s frog

Are the following amphibians in captivity?

Apparently three of the four salamander species are (or at least have been) held in several university labs, and some wild-caught individuals occasionally make their way into the hands of hobbyists. I'm not aware of any zoo collections you can see 'em at, though you can view Idaho giant salamanders at occasional open days hosted by the NNU Herpetology Lab.

Tailed frogs are also kept in labs (i.e. Washington State).

Darwin’s frogs can be seen at both the National and Concepcion Zoos in Chile. Here's a snap of the exhibit at the National Zoo in the ZooChat Gallery.

Where in the US are the following in public institutions?

Cuban Gar
Tropical Gar
a large collection of North American freshwater fish (darters, shiners, suckers, catfish)

I don't know about Cuban or Tropical Gar but lots of aquariums have "local" areas with fish native to that region. Shedd and the Tennessee Aquariums are two that spring to mind when talking about these; they have darters and I believe some of the other species you mention.
